Children & Youth Performance Outcome System Instruments (1998-2002)

Child Behavior Checklist for Ages 4-18 (CBCL):

Measures functioning on several dimensions including but not limited to functioning in school, home, and community settings; mental status; psychiatric disorders, and antisocial behavior from the parent's or caregiver's perspective. Youth Self Report for Ages 11-18 (YSR):

Measures functioning on several dimensions including but not limited to functioning in school, home, and community setting; mental status; psychiatric disorders, and antisocial behavior from the youth's perspective. Information about the YSR can be located in the Training Manual or on the ASEBA website.

Child & Adolescent Functional Assessment Scale for Ages 7-18 (CAFAS):

Measures youth functional domains including role performance in school/work, home and community; behavior towards others; moods; substance abuse, and thinking. Information about the CAFAS can be located in the Training Manual or on the CAFAS website.

Client Satisfaction Questionnaire (CSQ-8):

Measures consumer satisfaction with services received. Information about the CSQ-8 can be located in the Training Manual.

Client Living Environments Profile (CLEP):

Indicator of type of living situation and restrictiveness of the living situation. Information about the CLEP can be located in the Training Manual.

Family Empowerment Scale (FES):

Measures how a parent or caregiver of a child with an emotional problem feels about his or her role as a caregiver for the child. Information about the FES can be located in the Training Manual.

Youth Satisfaction Questionnaire (YSQ):

Children & Youth Performance Outcome Pilot Study Instruments (2000-2002)

From 2001-2002, the California Department of Mental Health, in association with the California Mental Health Directors Association and California Mental Health Planning Council, conducted a pilot test using the Ohio Scales for parent, child and clinician; a client information sheet and a more complete version of the CLEP called the Client Living Environment Stability Profile. The goal of this Pilot Study was to collect information on risk factors that would allow for a more accurate interpretation of outcome data as well as provide more information relevant to the entire children's system of care. Additionally, the pilot was conducted in English and Spanish.

The Children's Performance Outcome Pilot Study Protocols served as the guide for the design of the study.
